New Patient Visit

If you are preparing for your first visit to Dr. Kuhl's office, we look forward to meeting you. We want to help you create the most beautiful, healthy smile. We accept most major insurance plans and as a service to you, we will file your claims electronically right from our office. In your first visit, we will do an oral cancer screeing and complete exam including a video tour of your mouth. We will also introduce you to our friendly staff and explain all treatment options.

To make the most of your time in the office, you can get a head start by completing our New Patient Information form. Please download, print and fill out the three page form at your convenience. When you are finished, you may either bring them when you come to your appointment or fax them to us at 561-627-1569.
